We provide a series of high-quality ‘Professional Home Care Services’ packages for families with elderly members in need of care. During the domestic helper’s contract period, we can arrange for experienced nurses to provide the following services upon the employer’s request:

• Home visit support
• Telephone consultation support
• Home modifications and caregiving equipment recommendations
• Pre-discharge support

This ensures the home environment is safe and comfortable for the elderly to reside in, while also improving the domestic helper’s and cargiver’s skills in caring for the elderly. The goal is to enable the elderly to receive quality in-home elderly care.

1. Nurse Home Visit Support

We can arrange for experienced nurses to conduct home visits for elderly families in need. This allows the nurses to understand and assess the health condition and needs of the elderly individuals, and establish personalized care plans tailored to their specific needs. They also provide appropriate guidance to both the caregiver and the foreign domestic helper, ensuring the implementation of the care plan and ensuring that the elderly receive appropriate home care.

2. Nurse Telephone Consultation

Experienced nurses will also provide ongoing telephone-based professional consultation and support, offering advice on various caregiving needs for the elderly.
They will address questions and concerns from caregivers and foreign domestic helpers regarding the care of the elderly.

3. Home Modification Recommendations

Due to the decline in mobility and self-care abilities of the elderly, home modifications and purchase appropriate caregiving equipment and supplies are necessary to ensure their safety and comfort in aging at home. We will arrange for experienced nurses to conduct home environment assessments. They will provide recommendations to improve the environment and acquire caregiving equipment for the household, aiming to create a safe and comfortable living environment for the elderly and facilitate caregivers and foreign domestic helpers in providing more effective care.

We can also coordinate with renovation professionals to carry out home modification projects based on the recommendations of our nurse. This may include installing handrails, shower chairs, bathroom renovations, and other modifications to create a safe and comfortable living space for the elderly.

4. Pre-discharge Support

After elderly individuals are discharged from the hospital, their mobility and self-care abilities may decline, causing concerns and worries for their family members regarding their post-discharge care.
We can provide pre-discharge support for families in need. Experienced nurses will assess the elderly person and their home, and develop a comprehensive post-discharge care plan to provide full support for their various needs. 

5. Community Care Service Voucher Scheme for the Elderly

Starting from 2025, ‘WELL MATCH’ has become an approved service provider under the ” Community care service voucher scheme for the elderly” by the Social Welfare Department. We provide in-home services in all 18 districts across Hong Kong. With government subsidies, service voucher holders only need to pay a minimum of $280* per visit to enjoy the ‘Professional Home Care Services’ provided by ‘WELL MATCH’. These services include:
